Relevant Documents for Online Admission:

a) Naval Personnel: Form 1, Form 4,  DOB

Certificate,

b) Defence Civilian: Form 1, Form 4, DOB Certificate,

Proof of ID Card (ID Card to be shown at the time of

admission)

c) Ex-Service Men: Form 2, Form 4, DOB Certificate

and Discharge Sheet copy with the name of the child

mentioned in it.

d) For all category : Form 4 submission is mandatory
